HEM-POWR App and Website are intended solely for participants in the HEM-POWR study who reside in the United States: HEM-POWR is an Observational Study Evaluating Effectiveness and Safety of Real-World Treatment with Damoctocog alfa pegol in Previously Treated Patients with Hemophilia A.

The HEM-POWR APP AND WEBSITE ARE PROVIDED BY MICROHEALTH, INC, and they are subject to the Terms of Use and Privacy Policy of MicroHealth Hemophilia linked below, provided two exceptions:

1. Unlike MicroHealth Hemophilia, the HEM-POWR study app it is not intended for sharing information with your care team for care monitoring. Thus, HEM-POWR’s privacy policy restricts sharing of any information solely to the execution of the study (see “Purpose of the System”, below).

2. In the event that the MicroHealth Hemophilia terms of use might conflict with the informed consent provided to you to use the HEM-POWR App, the consent shall prevail. Thus, in case of withdrawal from the study, the terms provided to you as part of the informed consent prevails over MicroHealth Terms and conditions, added to this document for your reference.

Purpose of the System

The purpose of the HEM-POWR System is to help you to answer the electronic Patient-Reported Outcomes Questionnaires which are part of your participation in the HEM-POWR study. Utilizing Microhealth is absolutely voluntary and your decision to use or not use it will neither affect your participation in the HEM-POWR study nor the standard medical care for you / your child

This tool includes electronic reminders concerning Patient-Reported Outcomes Questionnaires, a personal and password-protected database for entry, and features to permit those involved in the HEM-POWR study to get relevant reports FOR NON-EMERGENCY REASONS ONLY.

The contents of this System, such as text, graphics, images, information and other materials contained on the HEM-POWR System ("Content") are meant to collect study from consented study participants. Any other information provided is for informational purposes only. The Content is not intended to be a substitute for professional medical advice, diagnosis, or treatment. Always seek the advice of your physician or other qualified health provider with any questions you may have regarding a medical condition.
